The CL ConnectedCare app is an easy-to-use remote monitoring solution for patients to share their health and outpatient recovery status in between follow-up visits. Patients are able to self-report their level of discomfort (on a scale of mild to severe), get reminders to take prescribed medication, and follow a structured exercise programme. The app is part of an integrated solution that includes a clock-face and a web dashboard for healthcare professionals to tap into a more holistic overview of the recovery process over a period of time. Through the web dashboard, clinicians can also assess the patient’s range of movement and alignment based on data, and review their Primary Score which helps to map their recovery trajectory.
